Job Description An employer in the OC area is seeking a Patient Care Coordinator. The coordinator is responsible monitoring for events, Gather Medical Records, Initiate Member outreach to engage in Complex Case Management Program and coordinator with RN Case Manager. Coordinator is responsible for mailing letters, educational materials, and community resources for the Member. The coordinator is also responsible for facilitating/coordinating patient appointments, benefits, and other healthcare needs.
